ES?L, AmpC and carbapenemase co-production in multi-drug resistant Gram-negative bacteria from HIV-infected patients in southwestern Nigeria

Background: The rising global emergence of Gram-negative bacteria (GNB) producing ?-lactam hydrolysing enzymes in clinical infections constitutes a growing public health threat. This study investigated the occurrence co-production extended spectrum ?-lactamase (ES?L), AmpC ?-lactamases, and carbapenemases among GNB isolated from HIVinfected patients two tertiary healthcare facilities southwest Nigeria.Methodology: A total 115 isolates previously recovered HIV-infected at Obafemi Awolowo University Teaching Hospitals Complex, Ile-Ife, State Specialist Hospital, Akure, were investigated. characterized to species level with Microbact 24E kit screened for ES?L production using double-disc test (DDT) combination disc methods, modified Hodge (MHT) EDTA disc, carbapenemase MHT test. Antibiotic susceptibility testing (AST) was performed by Kirby-Bauer diffusion method.Results: 15 characterized. AST profile revealed high resistance rates ampicillin (94.5%), tetracycline (74.5%), sulphamethoxazole-trimethoprim (66.3%), lowest imipenem (10.9%). Multi-drug (MDR) observed 93.6% while 98.8% ES?L, AmpC, carbapenemase-producing had multiple antibiotic (MAR) indices ? 0.2. detected 53.9%, 20.9% 25.2% isolates. or all three 80 (69.6%) isolates, only 10.0% produced enzymes.Conclusion: isolation MDR co-producing immunocompromised individuals portend grave consequences. Routine screening these will be highly essential guide institution appropriate therapy infection control measures.

 Conclusion: L'isolement co-producteurs d'enzymes ?-lactamines chez individus immunodéprimés laisse présager graves conséquences. dépistage systématique ces sera très essentiel guider mise place d'une antibiothérapie appropriée mesures contrôle infections.
